What is a consent form?

The consent form is a signed, optional statement stating that a patient agrees to the recording of their personal data and/or images for clinical and medical purposes. Consent forms are readily available for use in Celo:

Default Consent Form

Patients are free to agree or not whether they consent to the recording of their data and/or clinical images. Furthermore, patients may choose what purpose their data and/or images shall be used for: 

Clinical Record Only

The clinical images are for treatment purposes only, and will form part of the patient's confidential Clinical Record.

Restricted Educational Use

The images/video recording requested may be used both for the benefit of treatment and for medical teaching. They will form part of the patient's confidential Clinical Record.

Publication

The images/video recording requested may be used for the benefit of treatment and for medical teaching. It may also be published in a journal or textbook, form part of a display or information leaflet, or be included on a website. Any images used will not compromise the patient's identity unless otherwise specified.
