Android and iOS Only
Consent forms can only be attached using the mobile app (Android and iOS).
If you wish to attach a consent form to a photo or video, follow these steps:
Only the original uploader of a photo or video can attach a consent form.
- Open the photo or video you wish to attach a consent form to. You can look for it through the Library, or by opening up your photo or video from a conversation.
- With the photo or video open, press
to open Photo Details. - Ensure that a Patient ID has been specified for the file under Patient Details.
- Press
Add consent form. - You will be shown a list of your existing consent forms to attach to, and a choice to create a new one.
- To attach an existing consent form, select it and press Done.

- Open the photo or video you wish to attach a consent form to. You can look for it through the Library, or by opening up your photo or video from a conversation.
- With the photo or video open, press
